How do vocabulary gap-fill questions work?
One word is missing from a real sentence and four words could grammatically fit. Only one makes sense. These test whether you know what a word means in use, not whether you can recognise it in a list.
Why do the wrong options look plausible?
Because they are the same part of speech and the same level. A distractor you can rule out on grammar alone tests nothing. The intended difficulty is entirely semantic.
